PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation, interview and record review, the Hospital failed to ensure bed side-rails were only used when medically necessary and to eliminate an unnecessary ligature risk in the Patients' environment, for two (Patient #4 and Patient #7) of two sampled psychiatric patients in hospital beds.
Findings include:
On 4/17/19 at 10:30 A.M. the Surveyor toured the Hospital's North 5 Unit and observed medical beds with attached side-rails.
1. For Patient #4, the Hospital failed to evaluate the adaptive equipment to eliminate an unnecessary ligature risk in the patient's environment for the two bed side-rails attached to the Patient's bed.
The Surveyor observed two 1/2 side-rails attached to Patient #4's bed.
2. For Patient #7, the Hospital failed to evaluate the adaptive equipment to eliminate an unnecessary ligature risk in the patient's environment for the two bed side-rails.
The Surveyor observed two 1/2 side rails attached to Patient #7's bed.
The Surveyor interviewed the North 5 Nurse Manager on 4/19/19 at 11:00 A.M. The Nurse Manager said that the Hospital had not conducted an adaptive equipment assessment to determine the medical necessity and elimination of an unnecessary ligature risk with the use of side-rails for its current patients. The Nurse Manager said there were approximately eight beds which had side-rails attached to them.
PATIENT RIGHTS: RESTRAINT OR SECLUSION

Based on records reviewed and interview, the Hospital failed to ensure that all security staff, whose job requires assistance in the application of restraints, were certified in Cardiac Pulmonary Resuscitation (CPR) and first aid techniques.
Findings include:
The Hospital's Security Officer job description, undated, indicated that Security Officer's must have the ability to physically control a combative, assaultive and aggressive person and demonstrate appropriate skill level in the use of restraining devices and unarmed techniques.
The Surveyor interviewed the Security Site Supervisor at 11:30 A.M. on 4/19/19. The Security Site Supervisor said that the Security Guards employed by the Hospital assist with the application of restraints in the Emergency Department. The Security Supervisor said that the none of the Security Guards employed by the Hospital were trained in the use of CPR and first aid techniques.
