Based on observation and staff interview, the doors protecting the corridor openings of the facility were not maintained. Specifically, one of the corridor access doors was not positively latching when the door was closed.
The finding was:
On the afternoon of 5/6/2014 observation of the one hour fire-rated walls revealed that corridor access door for Room 148, the Value Room, did not positively latch when the door was closed.
The above finding was concurrently verified by Staff #11.

Based on observation and staff interview, the facility's sprinkler system was not properly maintained in accordance with NFPA 25, Standard for the Inspection, Testing, and Maintenance of Water-Based Fire Protection Systems. Specifically, the facility stored items less then 18 inches from a sprinkler head.
The finding was:
On the afternoon of 5/6/2014 observation in Social Worker Room 156 revealed that the gap between the top of some of the boxes and the ceiling sprinkler head was less then 18 inches. This hinders the function of the sprinkler head in the event of fire.

Based on observation and staff interview, a portable space heating device was found in a health care occupancy within the facility.
The finding was:
On the morning of 5/9/2014 observation in the Building #5 revealed that there was a portable space heating device found in the Doctor's Office.

Based on observation and staff interview the facility, flammable and combustible liquids were not stored in approved cabinets in accordance to NFPA 30, Flammable and Combustible Liquids Code.
The finding was:
On the afternoon of 5/8/2014 observation revealed that there was an unsecured, i.e. unlocked "fire-rated" cabinet in the Building #4 Outpatient Treatment Area. Within this cabinet there were two containers of Dural Quick Service Rubber Cement, sixteen cans of Tandy's Pro Dye Oil Based Paint, nine Cans of Oil and Latex Brush Cleaner, and seven cans of miscellaneous wood stain.
The above finding was concurrently verified by Staff #11.
