Based on observations, the facility failed to protect hazardous area in accordance with NFPA 101 sections 19.3.2.1. This deficiency practice affected the entire facility on the day of survey.
Findings include:
During the building inspection on 5/22/23 between 11:30 AM and 3:20 PM observation revealed the following:
Unsealed holes and missing ceiling tiles in Mechanical Rooms 1-3 of the facility.
The door to Storage Room 2056 lacked an automatic door closer.
The doors to Soiled Linen Room and Clean linen room near the (vacant) nurses ' station lacked automatic door closers.
The door to Storage Room 2057 lacked an automatic door closer.
All the findings in the areas above were incapable of resisting the passage of smoke throughout the facility.
