Based on document review and interview, the hospital failed to ensure an X-Ray report was not released, without authorization, outside the hospital for 1 of 1 released records (Record #1).
Finding:
On May 31, 2019, an email was received, from a complainant, in relation to Patient #1's care at the hospital. The email indicated that another patient's X-Ray report (Record #1) was contained in Patient #1's medical record when the complainant received it from the hospital.
On June 5, 2019, the complainant sent the surveyor an email with the other patient's X-Ray report attached.
On June 7, 2019 at 2:25 PM, a telephone interview was conducted with the complainant. The complainant verified she had sent the email to the surveyor with the other patient's X-Ray report and verified that this X-Ray was included in Patient #1's medical record when he/she received the medical record.
