Based on observation the facility failed to provide corridors that are clear and unobstructed for the complete width of the corridor for 2 of 5 halls in the facility. This deficiency affects all 10 patients in the facility.

Note: NFPA 101, 2000 edition
NFPA 101:7.3. The width of means of egress shall be measured in the clear at the narrowest point of the exit component under consideration. Exception: Projections not more than 3 1/2 in. on each side shall be permitted at 38 in. and below.

Findings:
During facility tour on February 6, 2012 between 1:00 p.m. and 3:30 p.m. there were carts, chairs and stands blocking the corridor in the following areas. Observations at 1:30 p.m., 2:00 p.m., 2:30 p.m. and 3:30 p.m. revealed that the chairs, carts and stands were left unattended in the same locations for more than thirty minutes.

1. There were 2 chairs left in the corridor across from the nurse station.
2. There were 3 medication carts left in the corridor by the pharmacy.

Items left in the corridors would prevent the rapid removal of patients in case of a fire or disaster.

Based on observation, the facility failed to provide a sprinkler system that is maintained and in accordance with NFPA 13:8.5.6. The clearance between the deflector and the top of storage shall be eighteen inches or greater. Items to close to the sprinkler heads would prevent the water from discharging properly causing the area not to be protected by the system. This deficiency could affect all 10 patients in the facility.

Findings:

During facility tour on February 6, 2012 between 1:30 p.m. and 3:30 p.m. it was observed that boxes of supplies and
food cans were closer than eighteen inches from the sprinkler head in the pantry of the kitchen area.
