Based on observation and interview, the facility failed to secure three of five medical gas cylinders in a fenced medical gas area outside of Obstetrics (OB) from falling whether connected, unconnected, full, or empty. The failed practice had the potential to affect all staff due to the potential for the cylinders to become a projectile if the cylinder heads were knocked off if the cylinders fell over. Findings follow:
A. While on tour of the facility on 06/19/18 at 2:40 PM, observation showed three unsecured medical gas cylinders in the fenced medical gas area outside of OB.
B. During the tour, the Services Director verified the failed practice.
