On the day of the recertification survey, based on observations and staff interviews, the facility failed to ensure that the rubbish chutes were maintained in accordance with NFPA 101 9.5.
Findings include:
Observation between 05-13-14 and 05-14-14 revealed that:
1. The trash chute doors on the 7th and 4th floors would not latch when tested.
2. The fusible link on the bio hazard chute door in the basement was bypassed with a wire loop.
The above items were verified by the Director of Engineering at the time of discovery. He had someone from his staff repair the items as we finished the tour of the facility.
