Based on review of facility documents and staff interview (EMP), it was determined the facility failed to ensure patient medical records were completed within 30 days following discharge for 268 of 292 applicable medical records reviewed (MR25 through MR292).

Findings include:

Review on January 15, 2019, of the facility's "GMC Rules and Regulations For Use Of the Medical Record" last revised and approved April 2018, revealed "... VII. Delinquent Medical Records Completion of the medical record is the ultimate responsibility of the attending physician and advanced practitioner (if involved) and should be done promptly at the time of discharge but in any event must be completed within 30 days of discharge. Resident and Advanced Practitioners are to complete medical record documentation within 10 days of discharge/service to ensure that the supervising physician can comply with state regulations for record completion. The Health Information Management Department will generate physician-specific, distribution by age, post-discharge reports of the number of incomplete medical records. In-Patient Records Providers (non-residents) identified by Health Information Management, having incomplete in-patient medical records more than 30 day post-discharge of the patient, will receive a notice of impending suspension (with a list of specific delinquent medical records) on Wednesday, after which the provider will have 7 days to complete the delinquent records. Failure to complete delinquent charts within the 7 day period with result in suspension of clinical privileges admitting, operating, consulting) until the records are competed. Providers with approved absence during the notification period will be exempt until returning, after which the impending suspension process will commence. Health Information Management staff will prepare and distribute the Suspension List to Admitting, Operating Areas ... As Health Information staff verifies medical records are completed, communication that a provider is removed from suspension will be distributed. ..."

On January 15, 2019, EMP1 provided "Delinquent Medical Record List by Provider". EMP1 confirmed this list as the delinquent medical record list greater than 30 days post-discharge of the patient.

1) Review on January 15, 2019, of the delinquent medical record list revealed OTH1 had 9 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH1's delinquent medical record list included the following:
MR25 was discharged on September 28, 2018
MR26, MR28, MR29, MR31 and MR32 were discharged on December 6, 2018
MR27 was discharged on December 5, 2018
MR30 was discharged on December 7, 2018
MR33 was discharged on December 3, 2018.

2) Review on January 15, 2019, of the delinquent medical record list revealed OTH2 had 1 delinquent medical record that was greater than 30 days from the patient's date of discharge. OTH2's delinquent medical record list included the following:
MR34 was discharged on December 12, 2018.

3) Review on January 15, 2019, of the delinquent medical record list revealed OTH3 had 1 delinquent medical record that was greater than 30 days from the patient's date of discharge. OTH3's delinquent medical record list included the following:
MR35 was discharged on August 19, 2018.

4) Review on January 15, 2019, of the delinquent medical record list revealed OTH4 had 1 delinquent medical record that was greater than 30 days from the patient's date of discharge. OTH4's delinquent medical record list included the following:

5) Review on January 15, 2019, of the delinquent medical record list revealed OTH5 had 9 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH5's delinquent medical record list included the following:

6) Review on January 15, 2019, of the delinquent medical record list revealed OTH6 had 8 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH6's delinquent medical record list included the following:

7) Review on January 15, 2019, of the delinquent medical record list revealed OTH7 had 5 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH7's delinquent medical record list included the following:

MR56 was discharged on October 27, 2018
MR57 was discharged on October 24, 2018
MR58 was discharged on October 31, 2018.

8) Review on January 15, 2019, of the delinquent medical record list revealed OTH8 had 3 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH8's delinquent medical record list included the following:

MR61 was discharged on November 16, 2018.

9) Review on January 15, 2019, of the delinquent medical record list revealed OTH9 had 6 delinquent medical records that were greater than 30 days from the patient's date of discharge. OTH9's delinquent medical record list included the following:
