Based on observation and interview, the facility does not maintain fire separations in the corridors.

Findings include:

Observation during tour on 7/22/10 at 10:00 AM revealed several penetrations in the corridor walls of the Medical Care Unit. Penetrations were found over the fire doors and between rooms #228 and 230.

These findings were verified with Staff #25 on 7/22/10.

Based on observation and interview, the facility does not provide fire safety to patients and staff on the hospital campus as related to unobstructed egress.

Findings include:

Observation during facility tour on 7/19/10 between 12:45 PM and 3:30 PM revealed storage of materials in exit stairwells. The penthouse stairwell landing was obstructed with boxes of communication system equipment, cafeteria stools, and a drafting desk with drawing storage. The penthouse mechanical room was cluttered with stored light fixtures and miscellaneous combustible materials. The Physical Therapy exit stairwell contained a closet that was constructed under the stairwell. (This closet is currently being removed.)

Based on observation and interview, the facility does not maintain the Fire Suppression Sprinkler System.

Findings include:

Observation during facility tour on 7/19/10 at 2:00 PM revealed no backflow device between the Fire Suppression Sprinkler System and the potable water supply. Continued observation at 3:00 PM revealed that two sprinkler heads in the purchasing department storerooms #1 and 4 were obstructed with suspended ventilation system ductwork.

Based on observation and interview, the facility does not provide fire safety for staff and for patients' medical records on the hospital campus.

Findings include:

Observation during facility tour on 7/20/10 at 12:00 PM revealed approximately 200 boxes of medical records stored in various areas of the facility's maintenance garage. This building, which is not protected with a fire suppression system, houses lawn mowers, a fork lift, hospital bed mattresses and other combustible materials. Two six-gallon gasoline cans and two 20-pound propane cylinders are stored in the building on the floor. Three 30-pound propane cylinders are stored outside the building on a cart in front of the overhead door.

These findings were verified with Staff #25 on 7/20/10.

THIS IS A REPEAT DEFICIENCY FROM THE SURVEYS COMPLETED 4/23/05 AND 4/11/08.
