Based on observation, interview, and record review, the facility failed to maintain peripheral intravenous catheter according to the facility policy for ten patients (#1, #2, #4, #6, #25, #26, #27, #28, #29 and #30) of 30 sampled patients. The facility census was 52.
Findings include:
Review of the facility policy titled "Intravenous Therapy," revised 10/2007, showed in part the following:
- Intravenous (IV) site will be rotated every 72 hours if possible or documentation of why not.
- Write on the tape the size of the jelco (catheter), date and time jelco started and initials of nurse who started the IV.
Observation on 12/7/10 at 9:20 AM, showed Patient #4's IV site was dated 11/28/10 (9 days old).
Observations on 12/7/10 between 10:00 AM and 10:45 AM, showed IV sites for Patients #1, #2, #6, #25, #26, #27, #28, #29 and #30 had no information written on the site to indicate date, time, jelco size or nurse.
During an interview on 12/8/10 at 10:25 AM, Staff P, Director of Nursing, stated IV sites are to be labeled. Staff P confirmed Patient #4's IV site had not been changed, and the medical record had no indication of why the site had not been rotated.

Based on observation, the facility failed to ensure that the environment was maintained by failing to remove dust from return air and supply vents for the heating and cooling system in patient care and hospital areas, and by failing to keep the floors and ice machines clean. The facility census was 52.
Findings include:
1. Observation on 12/7/10 showed that Critical Care Unit rooms 102, 103, 104, 105, 106 and 107 had dust on the majority of the surfaces of the return air vents. The vents had a metal cover flush with the ceiling and contained small holes for ventilation that were covered with dust or debris.
2. Observation on 12/7/10 in the Radiology area showed that in the room with the computerized tomography unit (CT or radiological imaging device), there was one supply vent and one return air vent. Dust covered the surface of both vents.
3. Observation on 12/7/10 of the second floor of the Behavioral Health Unit, nine of nine return air vents near the doors to the rooms of the adult and pediatric units were noted to have filters clogged with dust over the entire surface.
4. Observation on 12/7/10 of the Respiratory Therapy office and supply room showed that both rooms had a small return air vent that was covered in dust.

5. Observation on 12/08/10 at 8:00 AM, of the Kitchen area showed the floor tile behind the kitchen ice machine was chipped and loose. It was not a cleanable surface.
6. Observation on 12/08/10 at 9:00 AM, showed the ice machine on the first floor patient care area had black slimy residue in the corners of the drain tray.
