Based on observation and interview the facility failed to provide ground fault interruption (GFI) to electrical circuits throughout the kitchen. In addition the facility failed to provide identification for the breakers in one of three breaker boxes in the Kitchen area. This deficient practice affected all staff of the Kitchen. This facility had a census of 8.
Findings are:
1. Observations on 8/9/11 at 2:07 pm revealed the outlets located at all of the counter tops and throughout the kitchen failed to have GFCI [ground-fault circuit-interrupters] installed. Further examination of the panel boxes located in the kitchen area failed to reveal any GFCI breakers identified.
2. Observation on 8/9/11 at 2:08 pm revealed the identification tabs for the breakers in the KA Kitchen breaker panel box were missing.
Interview with Maintenance A on 8/911 at 2:09 pm confirmed the lack of GFCI ' s in the Kitchen area and the lack of the identification tabs in the KA breaker box.
NFPA Standard: (B) Other Than Dwelling Units. All 125-volt, single-phase, 15- and 20-ampere receptacles installed in the locations specified in (1), (2), and (3) shall have ground-fault circuit-interrupter protection for personnel:
(1) Bathrooms (2) Rooftops (3) Kitchens NFPA 70 210-8
NFPA Standard: All circuits and circuit modifications shall be legibly identified as to purpose or use on a circuit directory located on the face or inside of the panel door in the case of a panelboard, and at each switch on a switchboard. NFPA 70 408.4 Circuit Directory
