Based on interview and observation, the facility failed to provide adequate staffing as evidenced by the absence of a physician and nursing staff in the facility at the time of survey from at least 3/25/22 up until the date of survey.
Findings included:
In an interview on 5/3/22 at 9:45 am in facility's conference room during survey Entrance Conference in facility's conference room, Executive Director (ED)-Staff A and Director of Operations (DOO)-Staff B stated that the facility was not currently active. In addition, they stated there were no doctors and nursing staff present including Medical Director-Staff D, who they stated had remained until the last patient present was discharged.
In addition, during the Entrance Conference, ED-Staff A and DOO-Staff B stated the facility consisted of two units: Unit 2 and Unit 3. ED-Staff A stated that the last patient in Unit 3 was on 3/25/22.
In an interview on 5/3/22 at 11:00 in the facility conference room, Quality Assurance Director (QA)-Staff C stated Unit 2 discharged the last patient on 2/9/22.
Observation on 5/3/22 at 11:20 am of Units 2 and Unit 3 led by COO-Staff B and QA-Staff C showed both units were empty of staff and patients.
In an interview on 5/3/22 at 11:25 am while inside facility Unit 2, Staff A & B stated that they were unsure what the two units would look like in the future and they did not provide a timeframe as to when the units would be accepting new patients.
