RN SUPERVISION OF NURSING CARE

Based on staff interview and document review, it was determined the hospital failed to ensure a Registered Nurse (RN) appropriately supervised the care of each patient. Specifically, the RN failed to ensure proper bathing and hygiene care were provided to each patient in one (1) of eight (8) medical records reviewed in the survey sample. Medical record #1.

Findings:

Eight (8) medical records were reviewed 06/06/23 with the assistance of staff member #5 in the navigation of the electronic record. The medical record for patient #1 contained documentation that the patient was hospitalized February 26, 2023 to March 10, 2023 and that the patient required assistance from nursing staff to perform activities of daily living to include bathing, grooming, and hygiene. The only documented bath in the medical record of patient #1 was on March 9, 2023, day twelve (12) of a thirteen (13) day hospital stay.

An interview was conducted on 06/06/23 with an RN (staff member #8) working on the medical surgical floor that patient #1 was admitted to. Staff member #8 stated that baths should be given to patients daily.

An interview was conducted on 06/06/23 with a second RN (staff member #9) working on the medical surgical floor patient #1 was admitted to. Staff member #9 confirmed patients should be offered a bath daily. If a patient refused a bath, the patient should be offered a bath a second time during the shift and if the bath was again refused it should be noted in the medical record.

The medical record for patient #1 contained no documentation that a bath was ever offered and refused.

An interview was conducted with the education director on 06/06/23 who stated the hospital's expectation for bathing was to offer the patient, "hygiene" daily. The staff member stated full baths may be contraindicated due to special circumstances such as uncontrolled pain or end-of-life care. No documentation reviewed indicated that patient #1 had any contraindications to receiving a daily bath.

The above noted finding was discussed with the AVP of Quality at end of day on 06/06/23 and on 06/07/23 who confirmed the lack of documentation related to bathing patient #1.
