Based on observation and interview on 8-19-14 at 1:45PM, the facility failed to maintain the double doors for the Admissions Hall. Failure to ensure proper closure of these doors might allow the passage of smoke or fire. KCHS Hospital capacity is 10 beds and the census was 5 patients on 8-19-14.
Findings are:
Observations on 8-19-14 at 1:45PM, revealed the double doors for the Admissions Hall did not fully close during the closing tests and a gap remained after the doors tried to self-close.

Based on record review and interview on 8-19-14 at 4:00PM, the facility failed to have the fire alarm tested and inspected every 6 months as per NFPA 72, National Fire Alarm Code. Failing to test the fire alarm within this time frame may allow problems with the fire alarm system to go undetected. KCHS Hospital capacity is 10 beds and the census was 5 patients on 8-19-14.
