Based on document review and interview, the facility failed to ensure staff followed their policy/procedure for Patient Rights titled "Rights of Service Recipients".
Findings include:
1. Policy/procedure titled: "Rights of Service Recipients", Policy No: OP.09.01.00.00, revised/reviewed 04/2021 indicated on page 1: Each clients personal dignity shall be recognized and respected in the provision of all care and treatment. Page 3, 5. Each client's personal privacy shall be assured and protected within the constraints of the individual treatment plan.
2. Review of the Disciplinary Corrective Action Form dated 04/04/2023 at 1400 hours per staff A2 (Human Resource Generalist) indicated: N9 (Mental Health Technician) informed Registered Nurse (RN) that he/she recorded a client dancing in the hallway on his/her phone on 04/03/2023.
3. On 05/08/2023 at approximately 1500 hours, staff A1 (Chief of Utility Services) was interviewed and confirmed the facility staff should not record a video with a personal phone. Staff A1 confirmed facility staff should follow the policies/procedures for Rights of Service Recipients.

Based on document review and interview, the registered nurse failed to ensure the Nursing Assessment policy was followed for 2 of 10 inpatients (P4 and P6).
Findings included:
1. Review of the policy titled, "Nursing Assessment", Policy number: SOC # 3.2.0, revised/reapproved 10/16 indicated on page 1: Each patient admitted to the Inpatient Services will have a nursing assessment conducted by a registered nurse. The nursing assessment will be completed in total within 24 (twenty-four) hours of the patient's admission.
2. Medical record (MR) review indicated P4 was admitted to F1 on 03/07/2023 with a diagnosis of Schizoaffective disorder. The MR lacked a nursing assessment completed for this admission date.
3. MR review indicated P6 was admitted to F1 on 04/04/2023 with a diagnosis of Schizoaffective disorder. The MR lacked a nursing assessment completed for this admission date.
4. In interview on 05/08/2023 at approximately 1430 hours, A3 (Deputy Chief of Addiction Services) confirmed that the nursing assessment for P4 and P6 admissions listed above were absent from the MR.
