Based on observation, record review and interview, the facility failed to ensure that services were provided in a safe environment as evidenced by:
Failure to provide a safe non-ligature proof environment in 30 of 30 patient rooms (rooms
1601, 1602, 1603, 1604, 1605, 1606, 1607, 1608, 1609, 1610, 1611, 1612, 1613, 1614, 1615, 1701, 1702, 1703, 1704, 1705, 1706, 1707, 1708, 1709, 1710, 1711, 1712, 1713, 1714, and 1715) creating suicide risk and widespread endangerment to 17 of 17 patients (Patient #2, #3, #4, #5, #6, #7, #8, #9, #10, #11, #12, #13, #14, #15, #16, #17, and Patient #18).

Observation of 30 of 30 patient rooms on 12/31/2015 at 0930 revealed 48 of 48 patient beds with metal frames and springs that were not ligature proof.
Observation of 30 of 30 patient rooms on 12/31/2015 at 0930 revealed 30 of 30 patient bathrooms with non-ligature proof bathroom faucets.
In an interview with Personnel #51 on 12/31/2015 at 0830, she stated the census was 17. She stated the metal bed frames and faucets had been identified as ligature risks.
