Based on staff interviews, direct observation and review of facility maintenance and procurement records it was determined the facility failed to maintain storage cabinets and shelving in the surgery department.

The findings include:

During tours of the surgery and post-anesthetic recovery (PACU) areas, on 1/29/14 at 10:30 AM and 1/30/14 at 9:43 AM, it was noted there were upper and lower wooden cabinets affixed to the walls in various locations. These cabinets were observed to contain sterile and clean supplies as well as medications. The waterproof laminate that covered the doors and shelving was observed to be chipped, separated and missing. This was observed throughout the department. Laminate that was separated from doors in the PACU was observed to be patched with what appeared to be office tape. In the men's locker room, wooden cabinet shelving was repaired with what appeared to be medical tape.

A review of maintenance records revealed there were work orders for repair of cabinet laminate in the surgery suite that dated back to July of 2009.

Interviews were conducted with the Chief Nursing Officer, on 1/30/14 at 8:50 AM, and the Plant Operations Manager, on 1/30/14 at 10:55 AM. These interviews revealed the facility had planned on replacing cabinets and shelving throughout the surgical suite with metal cabinets. Although pricing had been obtained for the materials, no purchases had been made. The plant operations manager stated the labor to install the cabinets would be contracted out although no bids had been obtained. Additionally, no funds or capital expenditures had been approved for the project.
