Based on document review and interview, the facility failed to provide contract services in a safe manner by failing to provide facility specific orientation and patient rights training of (1) of (1) contracted housekeeper (HK1).

Findings include:
1. The facility was asked to provide documentation of facility specific orientation and patient rights training for a contracted housekeeper (HK1) involved in an altercation with a patient. No documentation was provided prior to departure.

2. Interview with S1, Director of Risk Management and Health Information Management, at 1500 hours on 7/27/2022, confirmed that HK1, Housekeeper, was a subcontractor to the facility's housekeeping contractor. No documentation was available to validate that HK1 had received facility orientation or patient rights training from the facility.

1. F1 "Patient Rights and Responsibilities" policy, PolicyStat ID: 11375046, last approved 4/2022, states that the patient has a right to be free from mental and physical abuse.

2. Incident report review indicating that on 6/28/2022, an altercation occurred between HK1 and patient P2 in which HK1 was terminated and escorted from the facility.

3. The facility was asked to provide documentation of facility specific orientation and patient rights training for a contracted housekeeper (HK1 )involved in an altercation with a patient. No documentation was provided prior to departure.

4. Interview with S1, Director of Risk Management and Health Information Management, at 1500 hours on 7/27/2022, confirmed that HK1, Housekeeper, was a subcontractor to the facility's housekeeping contractor.
