Based on observation, the facility failed to provide smoke barrier walls that were constructed to provide at least a 30 minute fire resistive rating for 2 of 2 smoke barrier walls in the facility. This could potentially affect 2 of 2 patients in the facility in the event of a smoke or fire emergency.
Findings: During tour of the facility on 11/18/10 between 8:00am and 11:30am, the smoke barrier walls next to the emergency department and by room 116 were observed to have several large unsealed penetrations in them with wires and conduit going through.
