Based on observation and interview the facility failed to maintain doors to ensure that they closed and latched according to Life Safety Code. This deficient practice has the potential to allow smoke and the spread of fire from storage rooms, and the spread into patient rooms in the 200 Wing. LSC 19.3.2.1, LSC 19.3.6.3 The facility is licensed for 25 beds.

Findings are:
1. During the inspectioni on 8-13-2013, with maintenance personnel, at approximately 1045 hours, the doors on storage rooms numbered 204 and 243 failed to close and latch properly. Interview with maintenance personnel verified that the doors should be adjusted to close and latch properly.

2. During the inspection on 8-13-2013, with maintenance personnel, at approximately 1100 hours, 6 of 10 patient room doors in the 200 Wing failed to close and latch properly. Interview with maintenance personnel verified that due to vinyl covering which had been installed on both the wooden doors and metal frames the doors did not close and latch.
