Based on observation and interview with staff (EMP) it was determined the facility failed to ensure sterile surgical supplies were stored in a manner to prevent contamination in 2 of 2 operating rooms observed (OR3 and OR5).
Findings include:
A request was made on August 21, 2017, at 10:00 AM to EMP1 for a policy regarding the storage and handling of sterile surgical supplies. None provided.
Observation on August 22, 2017, at 11:10 AM of OR3 revealed 2 sets of open shelving containing various sterile supplies used for the care of surgical patients.
Observation on August 22, 2017, at 11:10 AM of OR5 revealed 2 sets of open shelving containing various sterile supplies used for the care of surgical patients and 1 open shelved storage tower containing stored sterile and unsterile surgical supplies.
Interview on August 22, 2017, at 11:15 AM with EMPXXXX confirmed the sterile surgical supplies were stored in the OR3 and OR5 on open shelves and not protected from the surgical environment.

A request was made on August 22, 2017, at 2:15 PM to EMP 1 for a policy regarding the storage of clinical supplies. None provided.
Interview on August 22, 2017, at 11:15 AM with EMP1 confirmed the facility did not have a policy for the storage of sterile surgical supplies.
