PATIENT RIGHTS: GRIEVANCES

Based on document review and staff interview, it was determined that for 142 of 142 inpatients on 6/19/19, the hospital failed to provide the patient or the patient's representative with an accurate address to file a grievance with the State Agency. Findings included:

I. During an interview on 6/21/19 between 3:45 PM and 3:50 PM, Clinical Director A reported that patients were informed of their right to file a grievance with the State Agency in the following documents:
- "Rockford Center Responsive, Reliable, Respectful Behavioral Healthcare"
- "Rockford Center Children's Services, Parent Handbook"
- "Rockford Center Adolescent Services, Parent Handbook
- "Adult Services Inpatient Co-Occurring Treatment Program, Patient/POA/Guardian Handbook"

II. Review of the document entitled "Rockford Center Responsive, Reliable, Respectful Behavioral Healthcare" revealed the address listed for the State Agency was inaccurate.

III. Review of the following documents revealed no evidence that the information to file a grievance with the State Agency was included:
"Rockford Center Children's Services, Parent Handbook"
"Rockford Center Adolescent Services, Parent Handbook
"Adult Services Inpatient Co-Occurring Treatment Program, Patient/POA/Guardian Handbook"

This finding was confirmed by Clinical Director A on 6/21/19 between 3:45 PM and 3:50 PM.
