Based on document review and interview, it was determined, for 3 of 63 shifts from 9/1/14 to 9/22/14 on the male behavioral health unit (3 West), the Hospital failed to ensure staffing assignments were within an acceptable range.
Findings include:
1. On 9/23/14 at 1:00 PM, the Behavioral Health Units (BHU) staffing matrix, effective 7/1/14, for 3 West was reviewed. The BHU staffing matrix included, " RNs [Registered Nurses] 1 to 7/8 patients on 12 hour shifts. MHSs [Mental Health Specialists] 1 to 7/8 patients on 8 hour shifts. "
2. On 9/23/14 at 1:10 PM, assignment sheets from 9/1/14 through 9/22/14 for the Male Behavioral Health Unit (3 West) were reviewed. Staffing sheets on 9/5/14, 9/6/14 & 9/7/14 on the 7:00 AM to 7:00 PM shift, assigned 1 of 3 MHS to 13 patients.
3. On 9/25/14 at 10:30 AM, during an interview with the Chief Nursing Officer (E #6), E #6 stated he would have included the RNs in patient assignments, so 1 MHS did not have all the patients.
