Based on record review and interview, the facility failed to ensure 10 (#1, 2, 3, 4, 5, 6, 7, 8, 9, and 10) of 10 patients received patient rights information.
Findings include:
Review of the facility's admission paperwork revealed a document titled, "Patient Rights & Responsibilities." This document included the process for patients to file a complaint and/or grievance.
Review of the facility policy titled, "Complaint/Grievance Procedure," revealed the following: "All patients will be informed of their rights and responsibilities and provided with a copy of the Patient Bill of Rights, which includes the process for filing a complaint and/or grievance."
Review of patient charts revealed no evidence that any patient received the document titled, "Patient Rights & Responsibilities." There was no acknowledgement that the patient received this document on the general consent form or anywhere else in the record. Therefore, there was no evidence that patients were informed of the process for filing a complaint and/or grievance.
During an interview on 6/4/13 at 3:45pm in the conference room, staff #1 confirmed there was no documented evidence that any patient received the "Patient Rights & Responsibilities" document.
