Interior Wall and Ceiling Finish

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide documentation proving that the interior wall and ceiling finishes, including exposed interior surfaces of buildings such as fixed or movable walls, partitions, columns, and have a flame spread rating of Class A or Class B.

Fire Alarm System - Installation

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide a cover plate for a j-box that is part of the fire alarm system in the kitchen area.

Draperies, Curtains, and Loosely Hanging Fabr

The facility failed to provide documentation proving that the draperies, curtains including cubicle curtains and loosely hanging fabric or films shall be in accordance with NFPA 701 requirements.

Gas and Vacuum Piped Systems - Other

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide racks, chains, or other fastening to secure all cylinders from falling, whether connected, unconnected, full, or empty." - NFPA 99, 2012: 5.1.3.3.2
A single section of chain shall not secure more than two tanks.

Electrical Systems - Other

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide task illumination for the north generator and receptacles and task illumination for the south generator.

The life safety branch shall supply power for lighting, receptacles, and equipment as follows:
(4) Generator set locations as follows:
(a) Task illumination,
(b) Battery charger for emergency battery-powered lighting unit(s),
(c) Select receptacles at the generator set location and essential electrical system transfer switch locations. NFPA 99, 2012, 6.4.2.2.3.2

Electrical Systems - Maintenance and Testing

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide documentation showing that electrical receptacles are maintained of required tests and associated repairs of modifications, containing date, room or area tested, and results. 6.3.4 NFPA 99

Electrical Systems - Essential Electric Syste

The inspector observed, while accompanied by the maintenance assistant during the hours of the inspection from 1:00 pm to 2:30 pm on 5/3/2017 that there were the following issues. The facility failed to provide documentation showing that maintenance and testing of the generator and transfer switches are performed in accordance with NFPA 110. Generator sets are inspected weekly, exercised under load 30 minutes 12 times a year in 20-40 day intervals, and exercised once every 36 months for 4 continuous hours. Scheduled test under load conditions include a complete simulated cold start and automatic or manual transfer of all EES loads, and are conducted by competent personnel. Main and feeder circuit breakers are inspected annually, and a program for periodically exercising the components is established according to manufacturer requirements. Written records of maintenance and testing are maintained and readily available. 6.4.4, 6.5.4, 6.6.4 (NFPA 99), NFPA 110, NFPA 111, 700.10 (NFPA 70)
