Based on record review and interview the hospital failed to ensure that medical records were complete.
Findings include:
Review of the medical records report dated 3/10/15 and entitled "Incomplete Medical Records With Deficiency Over 30 Days" the hospital has an excess of 176 medical records at the time of survey that had not been completed. There are numerous records that are 100's of days over completion.
Interview with Staff A (RN) on 3/11/15 stated there is currently no administrative process for closing records.
