Based on observation and interview the facility failed to ensure that smoke barriers are maintained to be continuous and could prevent the passage of smoke.

Observation during tour on 3/11/15 between 8:00 a.m. and 4:00 p.m. with Staff A (Director of Engineering), Staff B (Director of Plant Operations) and Staff C (Emergency Preparedness & Safety Coordinator) revealed that the 4th floor visitors room (located in the connecting wing) near the main hospitals 4th floor east wing has 2 small penetrations above the suspended ceiling, next to the HVAC ductwork. These penetrations were observed in the visitors room (the connecting wing) and the 4th floor east utility room (main hospital).

Interview with Staff A and Staff B at the time of observation confirmed the finding and locations.
