Based on record review and interview, the facility failed to review and update the emergency preparedness plan annually. This deficient practice affected 5 of 5 residents, who currently reside in the facility, as identified by the administrator on 04/23/25. The facility had a census of 5 residents. Findings:

1. The facility records were reviewed 4/23/2025 at 9:30 a.m.. The administrator provided an emergency preparedness policy binder. The policy had not been reviewed and updated within the past 12 months.

2. The administrator was interviewed on 4/23/2025 at 10:00 a.m., and he stated he would review and update the emergency preparedness book.
