Based on observation and interviews, the facility failed to maintain interior finishes in accordance with manufacturers recommendations and specifications. This in the event of a fire could accelerate the development of the fire, somke and fire gasses, endangering the occupants of the building.

Findings include:

On July 8, 2013 at 3:12 pm while on tour of the facility with facility staff in the 4 tower West family waiting area mechanical closet, "R-Matte Plus-3" rigid insulating sheathing was observed attached to the walls. Interviews with staff indicated that it was installed to reduce sound transmission into the waiting room. According to the manufacturers instructions, " R-Matte Plus-3 MAY NOT be left exposed in any type of application." According to NFPA 101 (2000 edition) 10.2.4.3; "Cellular or foamed plastic materials shall not be used as interior wall and ceiling finish.Exception No. 1: Cellular or foamed plastic material shall be permitted on the basis of fire tests that substantiate their combustibility characteristics for the use intended under actual fire conditions"

Based on observations, the facility failed to provide maintenance and testing of the fire sprinkler system in accordance with NFPA 25. Failure to maintain and test system components could result in the system failing to perform as designed.

Findings include:

On July 8-9, 2013 while on tour of the facility with facility staff, pressure guages for the fire sprinkler system were observed to be beyond the required replacement or recalibration dates. These gauges were located in the following locations:
1) Floor 5 stairwell 3 gauge dated 05/08
2) Floor 3 stairwell 3 gauge dated 05/08
3) Floor 4 east tower gauge dated 2007
According to NFPA 25 (1998 edition) 2-3.2' "Gauges. Gauges shall be replaced every 5 years or tested every 5 years by comparison with a calibrated gauge. Gauges not accurate to within 3 percent of the full scale shall be recalibrated or replaced."
