Based on observation and testing, the facility failed to provide shaft partitions that have a fire resistance rating of at least one (1) hour.
Findings include:
While inspecting vertical openings on August 29, 2011 through September 1, 2011 the surveyor observed deficient items in the following vertical shafts:
1. Dumbwaiter shaft #16 and #17 that extend 4 floors had penetrations of the shaft walls throughout the entire shaft height.
2. Elevators #9, #10, #11, #12, #13 and #14 have penetrations around the control panels on the back wall of shafts on all floors.
3. All the ventilation shafts had small penetrations throughout all floors of the building.
4. Pipe chases had small penetrations throughout all floors of the building.
The Director of Operations and Administration was aware of these deficiencies and had contracted a company to repair all the vertical shafts prior to the survey. The work had not been completed at the time of the survey.
These deficient practices have the potential of effecting the entire facility.
