Based on medical record review, staff interview and review of the facility's policy, the facility failed to inform the patient of their rights in advance of furnishing care for 3 of 11 sample patients (#5, #9, #10). The findings were:

1. Review of the electronic medical record for patient #5 showed an admission date of 2/14/24 to the medical unit and a discharge date of 2/20/24. Further review showed the absence of a "Conditions for Registration/Services/Admission" document that included an acknowledgement of the notice of patient rights.

2. Review of the electronic medical record for patient #9 showed an admission date of 1/11/24 to the medical unit. Further review showed the absence of a "Conditions for Registration/Services/Admission" document that included an acknowledgement of the notice of patient rights.

3. Review of the electronic medical record for patient #10 showed an admission date of 2/16/24 to the medical unit. Further review showed the absence of a "Conditions for Registration/Services/Admission" document that included an acknowledgement of the notice of patient rights.

4. Interview with the patient access manager on 3/12/24 at 10:30 AM verified patients #5, #9 and #10 had been admitted to the hospital on the dates referenced above and none of the 3 patients had signed a Conditions for Registration/Services/Admission notice that acknowledged they were given notice of their patient rights.

5. Review of the facility's policy "Patient Rights and Responsibilities'' revised 08/30/23 showed..."B. Every patient, or when appropriate their representative, is provided a notice for their rights and responsibilities...D. Receipt of the Patient Rights and Responsibility Notice is documented in the patient's electronic Medical Record."
