Based on observation and review of medication package inserts and interview, the Facility failed to ensure outdated medications were not available for patient use. In addition, the Facility failed to ensure medications were stored according to manufacturer's guidelines. Findings follow:

A. Medication Room #1 was toured at 1320 on 11/28/12. Seven of eight bottles of Acetylcysteine 20% 30 ml (milliliter) were noted to have expiration dates of 09/12. In addition, six ampoules of Desmopressin Acetate 4 mcg/ml (microgram/milliliter) and four bottles of Succinylcholine Chloride 200 mg (milligram) 20mg/ml were noted to be frozen within a tray of ice. The thermometer in the refrigerator read 44 degrees.

B. Review of the package inserts for the Desmopressin Acetate and Succinylcholine Chloride revealed the medications were to be stored in a refrigerator at temperatures of 36 to 46 degrees.

C. The above was verified by the Executive Director of Nursing for Excellence at 1330 on 11/28/12.
