Based on document review and interview, there is no evidence to indicate all patients are entered into the central log and screened to determine if an emergency medical condition exists.

Findings include:

-Review of the Emergency Department (ED) log and Emergency Medical Services (EMS) patient care report (PCR) revealed the facility did not comply with all of the provisions of maintaining a central log and providing a medical screening exam on all individuals presenting to the ED. Please reference findings under Tag A2405 and A2406.

Based on document review and interview there is no evidence all patients presenting to the ED seeking assistance are entered into the central log. (Patient #1)

Findings include:

-Review of the EMS PCR revealed Patient #1 was transported to the ED via ambulance on 6/11/10 at 11:37am.
-Review of the ED daily log revealed no evidence Patient #1 was entered into the log on arrival to the ED.
-Interview with Staff #2 on 10/13/10 and Staff #5 on 10/14/10 revealed they had no recollection of Patient #1 presenting to the ED on 6/11/10.
-Interview with Staff #1 and #6 on 10/12/10 revealed the hospital had no documentation of Patient #1's arrival in the ED on 6/11/10 at 11:37am.

-Review of the ED daily log revealed no evidence Patient #1 was entered into the log on arrival to the ED.
-Interview with Staff #2 on 10/13/10 and Staff #5 on 10/14/10 revealed they had no recollection of Patient #1 presenting to the ED on 6/11/10.
-Interview with Staff #1 and #6 on 10/12/10 revealed the hospital had no medical record or other documentation of Patient #1's ED visit on 6/11/10 at 11:37am.
