During the tour of the facility conducted by the Fire Marshal's Team accompanied by Staff Members from Saint Joseph ' s it was noted that the facility failed to ensure that fire proofing for structural steel was maintained.
During the survey between 8 AM and 4 PM it was observed that the spray on fire proofing had been damaged in the 5th floor west mechanical room.

b.) 3rd floor provide an automatic closer on door for the storage room in Hospitalist area.

e.) 6th floor janitor closet seal holes at ceiling
f.) 4th floor mechanical room holes noted
g.) all soiled utility rooms must have positive latching doors

During the survey between 8 AM and 4 PM it was observed that the stairways in the east and west towers have openings that allow a sphere greater than 4 inches to pass through which is greater than allowed.

During the survey between 8 AM and 4 PM it was observed that the following areas were not protected properly by required smoke detectors.
a.) 7th floor public waiting area in elevator area which is not in view of the nurses ' station is required to have smoke detection provided.
b.) 5th floor Hospitalist sleeping room is required to have a smoke detector powered by the buildings electrical system and having battery backup.
c.) 3rd floor Endoscopy unit patient waiting room which is not in view of the nurses ' station is required to have smoke detection provided.
d.) 4th floor waiting area the smoke detector is located too close to the HVAC diffuser.
e.) 2nd floor pharmacy storeroom detector is located too close to the HVAC diffuser.

During the survey between 8 AM and 4 PM it was observed that the following sprinkler items were noted.
a.) Boiler room, fire sprinkler riser green tagged but dated 2010
b.) Dry-system riser yellow tagged.
c.) Main air handler area/obstructed fire sprinklers
d.) Material management area/ under electrical conduit & AC Duct.
e.) Exterior dock by gas room/if used for storage needs sprinkler coverage
f.) Main electrical room in dietary/no coverage
g.) Offices in Dietary/ no coverage
h.) Coolers in kitchen area/no coverage
i.) Green room/ sprinklers obstructed
j.) Riser room green tag has 4 different months punched for the same year.
k.) The fire pump room shall be one hour rated construction.
l.) All riser rooms and sprinkler zone valve locations shall be clearly labeled.
m.) Areas throughout the facility had storage
n.) Ensure that the sprinkler system in the penthouse area is in service.
