Means of Egress - General

Based on observation and interview, it was determined the facility failed to maintain means of egress in one location, affecting one of three floors.

Findings include:

1. Observation on October 22, 2019, at 10:50 a.m., revealed the second floor storage room aisleway was blocked by a large cart and precluded egress from one of two exits within the room.

Exit interview with the facilities manager on October 22, 2019, between 11:30 a.m. and 11:40 a.m., confirmed the means of egress deficiency.

Emergency Lighting

Based on documentation review and interview, it was determined the facility failed to maintain emergency lighting in one location, affecting one of three floors.

Findings include:

1. Observation on October 22, 2019, at 11:05 a.m., revealed the OR 5 emergency lighting fixture failed when tested.

Exit interview with the facilities manager on October 22, 2019, between 11:30 a.m. and 11:40 a.m., confirmed the emergency lighting deficiency.

Sprinkler System - Maintenance and Testing

1. Observation on October 23, 2019, at 10:20 a.m., revealed the exterior Fire Department Connection was blocked by shrubbery.

Exit interview with the facilities manager on October 23, 2019, between 11:20 a.m. and 11:30 a.m., confirmed the automatic sprinkler system deficiency.

Corridor - Doors

Based on observation and interview, it was determined the facility failed to maintain corridor openings in one location, affecting one of three floors.

Findings include:

1. Observation on October 22, 2019, at 10:30 a.m., revealed the third floor, Resident Room 107 door lacked smoke-tight integrity.

Exit interview with the facilities manager on October 22, 2019, between 11:30 a.m. and 11:40 a.m., confirmed the corridor opening deficiency.

Fire Drills
