Based on observation, interview and document review, the facility failed to follow their policy and procedure for Pediatric (Broslow) Crash Cart daily inspections, resulting in the potential for less than optimal outcomes for all pediatric patients treated in the emergency department, who require treatment and supplies from the Broslow Crash Cart.
On 3/27/2023 at 1150 while on tour of the Emergency Department (ED) with staff D and E it was observed that the Pediatric (Broslow) Crash Cart log, located on the top of the cart was not complete. Review of the February 2023 log titled "Broslow Cart" revealed the following dates were blank: 2/3, 2/4, 2/9, 2/10, 2/12, 2/15, and 2/21 through 2/27. Review of the March 2023 log titled "Broslow Cart" revealed the following dates were blank: 3/2, 3/4, 3/5, 3/11, 3/14, 3/17, 3/22 and 3/25 through 3/27.
On 3/27/2023 at 1215 during interview with staff D and E about the expectation of completion of the log for the Pediatric (Broslow) Crash Cart staff stated "I'm not sure what happened to this log, it should be on here and complete. The current log is not sufficient".
On 3/27/2023 at 1530 Review of the facility policy/procedure titled "Crash Cart/Defibrillator Check/Emergency Equipment Exchange" dated effective 11/2022, documented "The integrity of the Crash Carts/intubation boxes/respiratory boxes/emergency drug drawers should be maintained by documentation of inspection each business day or after each use. Inspections, including the defibrillator check should be documented on the appropriate crash cart log following directions on the log. Integrity is determined by an intact lock (red or yellow) and recording of the expiration date ...".
