Stairways and Smokeproof Enclosures

Based on observations, the facility failed to properly protect stairways in accordance with NFPA 101 section 7.2.3.3.1. This deficiency affected one (1) of five (5) smoke compartments exits in the facility on the day of survey.
Findings include:
On August 1, 2017 at 11:30 AM, observation revealed unsealed and open penetrations around the sprinkler pipes in the 2nd Floor Stairwell near the Sterile Supply Room.
Aisle, Corridor, or Ramp Width

Based on observation, the facility failed to provide clear and unobstructed corridors in accordance to NFPA 101 section 19.2.3.5. This deficiency affected one (1) of five (5) smoke compartments exits in the facility on the day of survey.
Findings include:
On August 1, 2017 at 11:15 AM, observation revealed the corridor near the O.B. Waiting Room was blocked and obstructed by locked traffic control doors. These traffic control doors failed to release upon activation of the fire alarm system.
Discharge from Exits

Based on observation and interviews, the facility failed to properly maintain exit egress as per Survey &Certification Letter 05-38. This deficiency affected one (1) of eight (8) exits in the facility on the day of survey.
Findings include:
On August 1, 2017 at 1:55 PM, observation revealed the exit discharge from the Maintenance Area lacked all weather, travel surface leading to the public way. This exit discharge also had the potential to be blocked and obstructed without all weather, travel surface.
