Based on observation, on the day of the survey, 6/22/2011, in the presence of Staff # 2 & #17, it was determined that the facility failed to maintain normally closed fire rated doors in the closed position.
Findings include:
1. In the former ground floor Laundry Area, now used primarily as a Storage Area a pair of fire doors was held open with storage boxes, in violation of NFPA 101 section 7.2.1.8.
