Based on observation the facility failed to provide adequate identification for existing rated walls and smoke barrier walls. As a general maintance schedule facility to verify all rated walls shall be properly identified.

All fire walls, fire barriers, fire partitions, smoke barriers, and smoke partitions shall be effectively and permanently identified with signs or stenciling per IBC 2012 section 703.7.


Based on observation the facility failed to provide adequate identification for existing sprinklers installed.

"A list of the sprinklers installed in the property shall be posted in the sprinkler cabinet." - NFPA 13, 2010, 6.2.9.7
"The list shall include the following:
1. Sprinkler Identification Number (SIN) if equipped; or the manufacturer, model, orifice, deflector type, thermal sensitivity, and pressure rating
2. General description (upright, pendent, residential, etc.)
3. Quantity of each type to be contained in the cabinet (min. of 2 of each type)
4. Issue or revision date of the list." - NFPA 13, 2010, 6.2.9.7.1

Portable Fire Extinguishers

Based on observation the facility failed to provide adequate fire extinguishers at kitchen.

"Class K Cooking Media Fires. Fire extinguishers provided for the protection of cooking appliances that use combustible cooking media (vegetable or animal oils and fats) shall be listed and labeled for Class K fires." - NFPA 10, 2012: 5.5.5

"Fire extinguishers installed specifically for the protection of cooking appliances that use combustible cooking media (vegetable or animal oils and fats) without a Class K rating shall be remove from service."- NFPA 10, 2012: 5.5.5.2.

In Kitchens: "A placard shall be conspicuously placed near the extinguisher that states that the fire protection shall be activated prior to using the fire extinguisher." - NFPA 10, 2012: 5.5.5.3

Electrical Systems - Other

Based on observation the facility failed to provide adequate identification for existing all essential system identifications including disconnect.


"Identification. All boxes and enclosures (including transfer switches, generators, and power panels) for emergency circuits shall be permanently marked so they will be readily identified as a component of an emergency circuit or system." - NFPA 70, 2011, 700.10.

"Circuit Directory or Circuit Identification. Every circuit and circuit modification shall be legibly identified as to its clear, evident, and specific purpose or use. The identification shall include sufficient detail to allow each circuit to be distinguished from all others. Spare positions that contain unused overcurrent devices or switches shall be described accordingly. The identification shall be included in a circuit directory that is located on the face or inside of the panel door in the case of a panelboard, and located at each switch or circuit breaker in a switchboard. No circuit shall be described in a manner that depends on transient conditions of occupancy." - NFPA 70, 2011, 408.4 (A) Note: Labeling includes all disconnecting means, see 110.22.


Based on observation the facility failed to provide adequate identification for existing fire alarm circuit.

"For fire alarm systems the circuit disconnecting means shall be identified as "FIRE ALARM CIRCUIT." - NFPA 72, 2010, 10.5.5.2.2

"For fire alarm systems the circuit disconnecting means shall have a red marking." - NFPA 72, 2010, 10.5.5.2.3


Based on observation the facility failed to provide adequate support of existing low voltage wires that were resting on the ceiling grid.

"Support of Conductors. Raceways shall be used for their intended purpose. Communications cables or wires shall not be strapped, taped, or attached by any means to the exterior of any conduit or raceway as a means of support." - NFPA 70, 2002, 800.52 (2)(E).

Based on observation the facility failed to provide adequate space for the existing deep fryers.

All deep fat fryers shall be installed with at least a 16 inch space between the fryer and surface flames from adjacent cooking equipment. 2001 NFPA 96, 12.1.2.4.
