B ased upon observations, the facility failed to maintain one (1) storage room in accordance with the 2000 edition of the Life Safety Code, NFPA 101 (National Fire Protection Association). Findings included:

Surveyor observed between 1:00 and 1:30 pm that first floor Linen room next to Women's toilet room and East Stair did not have self closing device as required for hazardous protection.

Based upon observations, facility failed to maintain building services in accordance with NFPA 101, 2000. Findings included:

Surveyor observed between 1:00 and 1:30pm on 9/22/10 that a ceiling leak was occurring in first Laboratory area.
