PATIENT RIGHTS: CARE IN SAFE SETTING

Based on interview and record review the facility failed to protect a patient from a physical assault causing injury for one of one patient with bruises and lacerations (#1) reviewed.

Patient #1, a 91 y/o male, oriented to self only with a past medical history of Heart failure, Diabetes, Glaucoma, Alzheimer's, Dementia and Mood disorder. Occurrence Report 2/29/24 at 06:49 by RN #P stated patient #15 stated patient #1 was coming into his room and refusing to leave despite "persuasion." Patient #1 was refusing "so he punched him in the face."

Occurrence Report documentation indicates attack occurred at 05:45 am, the prior 6pm to 6 am shift but was documented by staff from the oncoming shift. Actions taken post -incident has "Supervisor notified" marked. No physician or family notification was marked. No indication if the victim was injured was documented. No follow-up resolution was documented.

Progress Note 2/29/23 at 06:06 am by RN # R read patient was attacked on his forehead by a peer. Swelling noted on the left forehead. First aid care provided. Vital signs taken. Charge nurse made aware of incident.

Daily RN Shift Assessment dated 2/29/24 by RN #Q at 12:41 pm listed interventions as Fall precautions and Q 15-minute checks. Nursing and provider notes 2/30/24 indicate the roomate was moved to another room. Patient #1's record did include any safety interventions for wandering or hepling him identify his room form others.

When interviewed 4/4/24 at 11:45 pm staff # J, Registered Nursing stated every 15 minute rounds are adequate for monitoring all patients unless they are on 1:1 precautions.
