Based on observation, the hospital failed to assure 4 of 8 inspected outpatient locations have operational emergency exit lights. Findings include:

Per observation on 12/3/12, accompanied by the Director of Plant Operations, the following associated practices/outpatient locations required repairs to emergency exit lights in accordance with NFPA 1 section 10.4:

1. Middlebury Pediatric and Adolescent Medicine at 1330 Exchange Street in Middlebury;
2. Porter ENT located at 1330 Exchange Street in Middlebury;
3. Porter Internal Medicine at 116 Porter Drive in Middlebury;
4. Addison Family Medicine at 82 Catamount Park in Middlebury.

Based on observation, the facility failed to assure medical gas storage is in accordance with NFPA 99, Standards for Health Care Facilities in one area of the hospital.

Per observation on 12/3/12, accompanied by the Director of Plant Operations, there were 3 large oxygen tanks in the storage closet near the operating room entrance that were not secured to the wall and were found to be free standing. NFPA 99 section 5.3.13.2.2(11)

Based on observation, the facility failed to assure electrical wiring and equipment is in accordance with NFPA 70, National Electrical Code, in 2 areas of the facility and 1 of 8 outpatient areas inspected.

Per observation on 12/3/12, accompanied by the Director of Plant Operations, the following violations were observed:

1. An extension cord was being used as a replacement for permanent wiring in 1 office of the Business Building. NFPA 70 section 400.8(1)

2. An overhead projector cord was in an outlet above a suspended ceiling in 1 room. Per NFPA 70 section 400.8(2), cords cannot run from below a ceiling to above.

3. At the outpatient office of Addison Associates in OB/GYN located at 116 Porter Drive in Middlebury, the outlets in the employee kitchen area were not GFCI (Ground Fault Circuit Interruption) protected outlets as required by NFPA 70 section 210.8.b.
