Based on record review, policy review, and interview, the facility failed to provide appropriate social work supervision and oversight for the biopsychosocial assessments for seven (7) of seven (7) active sample patients (Patients 1, 2, 3, 4, 5, 6, and 7). This deficiency potentially impacts the patient's overall treatment and may delay discharge.
A. Record Review
The following biopsychosocial assessments were not completed by social work staff and were not reviewed or signed by a Master's prepared social worker:
Patient 1, biopsychosocial dated 5/30/18, was completed by the social services coordinator who was not MSW prepared.
Patient 2, biopsychosocial dated 5/31/18, was completed by the social services coordinator who was not MSW prepared.
Patient 3, biopsychosocial dated 6/2/18, was completed by a psychology intern and not cosigned by a social worker.
Patient 4, biopsychosocial dated 6/2/18, was completed by a psychology intern and not cosigned by a social worker.
Patient 5, biopsychosocial dated 6/4/18, was completed by a psychology intern and not cosigned by a social worker.
Patient 6, biopsychosocial dated 6/4/18, was completed by a psychology intern and not cosigned by a social worker.
Patient 7, biopsychosocial dated 6/4/18, was completed by a psychology intern and not cosigned by a social worker.
B. Policy Review
The facility policy titled, "Inpatient Intake," and revised 11/21/14 stated, "The inpatient case manager will be responsible to obtain the information from the client. However, if the case manager is not available to obtain the information due to a client's hospitalization that is less than seventy-two (72) hours, the intake information may be obtained by the registered nurse." This policy did not ensure that all biopsychosocial assessments were developed or reviewed by a social worker.
C. Interview
On 6/5/18 at 11:30 a.m., the Director of Social Work confirmed that the biopsychosocial assessments were not co-signed or reviewed by a Master Prepared Social Worker and that the current facility policy did not state that the biopsychosocials must be completed by a qualified social worker.
