Based on document review, observation and interview, it was determined the facility failed to ensure supplies commonly used in life-saving procedures, including pediatric emergency system packets were not available for use past the expiration date for 1 of 1 Emergency Departments (ED) and to ensure documentation of maintenance of equipment used for therapeutic interventions, including hydroculator and freezer packs was current for 2 of 2 (Occupational Therapy and physical Therapy) storage areas reviewed.

The findings included:

1. Observations in the Emergency Department on 11/13/14 at 8:30 AM revealed the following:
One pediatric emergency system packets (which included catheter/needle, intravenous prep kit, and extension set) with an expiration date of 5/2014.
Two pediatric emergency system packets with an expiration date of 8/2014.

During an interview in the ED on 11/13/14 at 8:35 AM, Registered Nurse (RN) #1 verified the pediatric emergency system packets were past the expiration date.

2. Observations in the occupational therapy area on 11/12/14 at 9:45 AM revealed a "Weekly Temperature Assurance" log with the last temperature documented for hydrocollator #2 as 5/13/14.

Observations in the physical therapy area on 11/12/14 at 9:48 AM revealed a "Weekly Temperature Assurance" log with the last temperature documented for hydrocollator #1 and the cold pack freezer as 12/11/13.

During an interview in the physical therapy area on 11/12/14 at 9:50 AM, the Physical Therapist stated he had added water to hydrocollator #2 last week. When he was asked if he checked the temperature, he stated yes, "...I know I'm [I am] supposed to write them down..."
