Based on observation, documentation review, and staff interview, the facility failed to ensure 4 staff members working in the Kitchen/Cafeteria grill stations washed their hands when required by State law.
Findings included:
1. At 12:15 PM on 3/17/2014, staff member #6 was observed changing his/her gloves multiple times without washing his/her hands between the changing of the single-use gloves. Another kitchen staff member working on the patient line as a grill cook was observed changing his/her gloves after soiling them. The staff member did not wash his/her hands before putting on the new pair of single-use gloves.
2. At 1:10 PM on 3/17/2014, two Cafeteria grill station staff members were observed not washing their hands between changing of gloves multiple times.
3. 410 IAC 7-24-129, Retail Food Establishment Sanitation Requirement, When to Wash Hands, states, "Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation, including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and the following: After touching bare human body parts other than clean hands and clean, exposed portions of arms; After handling soiled surfaces, equipment, or utensils; During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; When switching between working with raw food and working with ready-to-eat food; Before touching food or food-contact surfaces; Before placing gloves on hands; and After engaging in other activities that contaminate the hands."
4. At 12:45 PM on 3/17/2014, staff member #6 indicated he/she has handled raw hamburger patties and raw fish with his/her gloved hands and continued grilling at the grill station. The staff member said he/she has handled the buns with the same gloved hands.
5. At 2:35 PM on 3/17/2014, staff member #3 confirmed that the staff was not washing their hands between changing of gloves.
