PATIENT RIGHTS: CARE IN SAFE SETTING

Based on observation, the facility failed to provide a safe environment for 1 of 3 patients (Patient #20) with safety bed alarms on the Behavioral Health Unit (BHU).

Findings include:

1. During tour of the BHU beginning at 10:40 a.m. on 3/9/10, the following was observed:
(A) Three (3) patients had safety bed alarms on their beds. The alarms had a pad that is placed under the patient and an alarm box that sounds when the patient rises from the bed to alert staff that the patient is up.
(B) Patient #20 was observed rising from his/her bed with a safety bed alarm on the bed. The alarm did not sound.
(C) Staff did not check the status of the alarm until asked by surveyor. At that time, the alarm was checked by staff member #N1 and found to have a portion of the cords pulled apart making it nonfunctioning.
