Based on observations made during a tour of the facility on 1/5/11, it was determined that the facility failed to ensure that the hazardous area doors automatically close a latch as required.
Failure could allow fire, smoke and toxic gases to escape that room in the event of fire.

The findings include:

The soiled utility/biohazardous waste collection and storage room located on the 2nd floor, at the nurses ' station was observed to have the striker plate opening filled with cardboard and the door latch held in the open position with medical tape. This prevented the door from latching so that the staff could just push on the door as opposed to turning the knob.

Based on a review of the facility records and interview with the Plant Operations Director during the Fire Life Safety survey on 1/3/11, it was determined that the facility failed to ensure that all Essential Electrical Equipment was tested as frequently as required. Essential Electrical Equipment not tested as frequently as required renders the equipment unreliable and undetected faulty equipment could cause the safety features of the equipment to fail.

The findings include:

There was no documentation at the time of the survey to show that the Line Isolation Monitor Test Instruments were tested annually as required. The Plant Operations Director determined that there was a mistake where two departments were under the impression that the other was meeting this requirement.
