K200 Means of Egress Requirements - Other
Findings:
The inspector observed, while accompanied by the Hospital Operations Manager & Maintenance Supervisor, during the hours of the inspection from 11:45 am - 3:45 pm, that there were the following issues:
1) Based on review of the records, facility failed to perform annual inspection of doors in means of egress since the last LSC survey in 2016.

K914 Electrical Systems - Maintenance and Testing
Findings:
The inspector observed, while accompanied by the Hospital Operations Manager & Maintenance Supervisor, during the hours of the inspection from 11:45 am - 3:45 pm, that there were the following issues:
1) Based on review of the records, facility failed to do the following: Hospital-grade receptacles at patient bed locations, are tested after initial installation, replacement or servicing at intervals not exceeding 12 months.

K918 Electrical Systems - Essential Electric System Maintenance and Testing
Findings:
The inspector observed, while accompanied by the Hospital Operations Manager & Maintenance Supervisor, during the hours of the inspection from 11:45 am - 3:45 pm, that there were the following issues:
1) Based on review of the records and interview with Hospital Operations Manager & Maintenance Supervisor, facility failed to perform load bank test for four continuous hours every 36 months.
