Based on document review and interview the facility failed to ensure a patient's grievance was investigated/addressed for one (1) of ten (10) medical records(MR) reviewed (Patient # 10).
Findings include:
1. The hospital policy titled, "Patient/Family Grievances", PolicyStat ID 13420947, indicated at each level of the grievance process the hospital staff shall listen to the patient's grievance, assure the patient that his/her concern will be investigated, that the grievance will be investigated and addressed within 24 hours.
2. Review of patient # 10 medical record indicated Seclusion/Restraint Order dated 4/17/2023 indicated the patient was punching walls, banging face into ground, and hitting windows. This patient reported that staff threw patient # 10 on the bed and pushed him into plastic shelf in room hitting his/her left collar bone area, patient # 10 complained of increased pain and decreased ability to move left arm.
3. In telephone interview on 7/7/23 at approximately 10:40 am with administrative staff member A # 1, confirmed there was no investigation into the complaint allegations made by patient # 10 on the restraint /seclusion order from 4/17/2023.
