Based on interview, policy review and the review of the clinical record for one of one patient named in the complaint, there were 52 verbal and/or telephone orders and 52 of the 52 orders failed to be timed and dated as to when the Physician(s) authenticated these orders.
Findings were:
Based on the review of the clinical record of the only patient named in the complaint, there were 52 verbal and/or telephone orders in the patient record. None of the 52 of 52 physician orders were timed and dated to verify that the physician(s) authenticated these orders within 48 hours as required.
The above finding was confirmed in interview with the Nursing Director and QA/risk manager (at the time of the incident) in the board room on 6/22/10 in the afternoon.
